Nurse Practitioner Coordinator- Lung Cancer Screening Program
Full-TimeJamaica Hospital Medical Center a member of the Medisys Health Network is a 404 bed fully accredited community-based teaching hospital and Level I Trauma and Stroke Center. Jamaica Hospital Medical Center has a great opportunity for a Nurse Practitioner Coordinator for the Lung Cancer Screening Program. This position is a non-union role.
Job Requirements:
1. Working with Epic/IT to develop and/or implement:
- Order set for Low-Dose Computed Tomography (LDCT)
- Data Tracking Software
- Data collection and reporting on all aspects of program including patient outcomes.
- Structured reporting system for the Lung Cancer Screening Program
- Needed forms
- Required reporting to governmental or supervising medical agencies
2. Collaborating with the Department of Radiology to assure compliance with ACR-STR Technical Specifications for LDCT and required reporting
3. Enter patient data into tracking software and use system to monitor and follow up patients. (This will cover all aspects of the program including nodule detection rate, lung cancer diagnoses, procedures, complications, outcomes, etc.)
4. Patient care activities
- Schedules/Coordinates appointments for the Shared Decision-Making Visit (SDMV), CT scan, follow up visits, etc. Reminders (calls, letters, e-communication) to patients.
- Obtains authorizations for scans when required
- Sends out letters, and/or e-communications to patients and physicians regarding results of testing and recommendations for follow-up. Follows up with telephone calls as needed
- Screens all patients for whom testing is ordered, or are referred to the program, for eligibility for a screening scan (correct number of pack years, age, etc.)
- Performs SDMV, and documents attestation in the chart
- If SDMV performed by another provider, ensures that appropriate document is in the chart
- Coordinates Multidisciplinary group that reviews results and makes recommendations
- Ensure patient f/u of incidental findings
- Ensure patient f/u with additional studies/treatment required for nodules (PET scans, biopsies, oncology consults, etc.)
- Ensure yearly follow up by tracking and communicating with patients.
5. Providing and developing materials for patient and provider education
- Educating providers so that they can adequately discuss benefits and harms of screening with their patients
- Collaborating with Epic to incorporate available standardized education materials into the EMR to assist Providers in patient education
- Overseeing and supplementing Provider-based patient education
6. Integrating the Lung Cancer Screening program with the smoking cessation program
- Facilitating referrals to the Freedom from Smoking program
- Collecting data related to smoking cessation interventions that are offered to active smokers enrolled in the screening program
- Receive training in smoking cessation and provide counseling to current smokers referred to the program. A separate clinic can be created for this purpose.
7. Collecting data related to the quality of the Lung Cancer Screening Program, including data on compliance with program policies and procedures
- Reviewing data and supporting the development of quality improvement initiatives.
- Reporting summary of data collected to an oversight body as required.
- Reporting to supervising JHMC Leadership Committee
8. Ensuring that Policies and Procedures are up to date and in compliance with evidence-based guidelines and other oversight body requirements.
9. Participating in research activities.
