This position is inactive

The primary responsibility of the Patient Navigator is to act as a liaison between MediSys Health Care System and the patient. Patient Navigators are expected to provide continually exceptional customer service to the patient population and MediSys colleagues; outreach to the patient population; identify missing medical services and assist with the scheduling of these services; identify and problem solve barriers to keeping appointments; inform physicians of patients’ missing medical services, and assist patients’ on-site if needed. Additionally, Patient Navigators will be asked to participate in the Diabetes Prevention Program as a Lifestyle Coach, providing weekly educational support in a group setting to pre-diabetic patients. (Training provided by MediSys.)

Requirements
A minimum of an Associates’ Degree is preferred, but not mandatory. Alternatively, at least three years’ experience in community health is acceptable. Additionally, other health care certifications (e.g. Medical Assistant certification) preferred, but not mandatory. Ideal candidate would be bilingual / QMI (Qualified Medical Interpretation) certification a plus. Strong computer skills (EMR, Excel, Word, Power Point,)Excellent interpersonal skills. Excellent communication skills, both oral and written. Ability to work effectively in a team. Detail oriented. Speak and read languages common to the community. Medical experience preferred, but not mandatory (i.e. Medical Receptionist). Familiar with community resources and hospital processes, structure, and functions. Prior experience as a health educator or case manager preferred
