Requirements
What We’re Looking For
- Active NYS RN License (required)
- BSN required | MSN preferred
- 3+ years leadership or team lead experience in case management or care coordination
- Strong expertise in: Patient assessment & discharge planning
- Care coordination & utilization review
- Crisis intervention and problem-solving
- Excellent communication, organizational, and leadership skills
- EPIC experience preferred
