Case Management Supervisor (RN)
Full-Time | Jamaica Hospital Medical Center – Queens, NY
Salary: $160,000 + Comprehensive Benefits | 37.5 Hours/Week

Jamaica Hospital Medical Center, a 384-bed community teaching hospital serving one of New York City’s most diverse populations, is seeking a Case Management Supervisor (RN) to lead and elevate our Case Management program.

This is a high-impact leadership role where you will mentor a multidisciplinary team, drive quality outcomes, and directly influence patient care coordination across the continuum. If you’re an experienced RN leader who thrives in fast-paced healthcare environments and is passionate about improving systems and outcomes, we want to hear from you.

What You’ll Do

  • Lead & Develop Your Team
  • Supervise, mentor, and support a team of Case Managers
  • Foster professional growth, accountability, and best practices
  • Promote a collaborative, patient-centered culture

Oversee Clinical Excellence

  • Manage case assignments and review care plans
  • Provide guidance on complex, high-risk, or escalated patient cases
  • Participate in crisis interventions and interdisciplinary decision-making

Drive Operations & Quality

  • Ensure compliance with hospital policies, NYS regulations, and documentation standards
  • Analyze program performance data and identify improvement opportunities
  • Optimize workflows to enhance efficiency, patient outcomes, and discharge planning

Requirements

What We’re Looking For

  • Active NYS RN License (required)
  • BSN required | MSN preferred
  • 3+ years leadership or team lead experience in case management or care coordination
  • Strong expertise in: Patient assessment & discharge planning
  • Care coordination & utilization review
  • Crisis intervention and problem-solving
  • Excellent communication, organizational, and leadership skills
  • EPIC experience preferred
