In response to the increase in violence, Jamaica Hospital Medical Center, an American College of Surgeons Verified and New York State Designated Level 1 Trauma Center, located in South Queens, NY is committed to addressing the root causes of trauma recidivism that have plagued our community. The Violence Elimination and Trauma Outreach (VETO) Program is our Hospital-Based Violence Intervention Program (HVIP) serving victims of interpersonal violence in the community. We are seeking a full-time Violence Prevention Care Coordinator to implement case management services to VETO participants.

 

The Care Coordinator is a team member of the Trauma Division and will work under the supervision of the VETO Program Manager, Social Worker, Trauma Program Director, and the Trauma Medical Director. This is a grant-funded position with secured funding until June 30, 2025, with a strong possibility of extension.

 

Duties and Responsibilities

  • Work in collaboration with interdisciplinary team to identify care needs as directed; as well as identifying underdeveloped patient care needs.
  • Provide telephone outreach calls based on direct monthly registry list; (i.e. for high-risk patients to review plan of care as indicated based on established criteria.)
  • Collaborate with internal and external medical/mental health providers to support client compliance with, and timely completion of, wellness plans.
  • Schedule and escort patients to wellness-related appointments (medical, educational, employment, legal, health, social service, etc.) as needed
  • Establishes relationships as a liaison with external social service agencies.
  • Maintain accurate documentation of information (e.g., record of interactions, demographics, referral logs, etc.) regarding hospital patients, VETO participants, and their families.
  • Ensures continuity of care by making follow up contact with patient after provider visit using established guidelines and provider directives.
  • Attend appropriate community meetings and continuing educational opportunities.
  • Perform other duties as assigned by the Program Manager, Social Worker, Trauma Program Director, and Trauma Medical Director.

Requirements
  • Bachelor’s degree in social work or related human services field
  • Must have a working knowledge of social services community programs
  • Excellent time management skills and ability to meet short deadlines
  • Respect and empathy towards diverse client populations
  • Demonstrated ability to work independently and as part of a team
  • Ability to work in stressful situations

 

Preferred Qualifications

  • Intermediate level of proficiency with Microsoft Office Suite, EPIC,
  • Fluency in Spanish preferred; not required.
  • Knowledge of urban youth issues, specifically youth violence
