Diabetes Educator – Women’s Health Center

Jamaica Hospital Medical Center – Women’s Health Center
Salary: $116,000
Schedule: Monday–Friday, 9:00 AM – 5:00 PM

Jamaica Hospital Medical Center is seeking a dedicated Diabetes Educator to empower patients through education, lifestyle guidance, and personalized care strategies to manage their condition effectively.

RESPONSIBILITIES:

  • Educate patients on diabetes types, risk factors, and potential complications.
  • Provide instruction on blood sugar monitoring and management techniques.
  • Offer guidance on lifestyle modifications, including nutrition and physical activity.
  • Support safe and effective medication use, including insulin administration.
  • Assist patients in recognizing and managing hypo- and hyperglycemia.
  • Facilitate access to diabetes-related services, resources, and products.
  • Collaborate with healthcare teams to develop and implement individualized care plans.

Requirements
  • Education: Bachelor’s degree in Nursing, Dietetics, or Nutrition from an accredited institution.

Experience:

  • Minimum of two years of professional experience in diabetes care.
  • At least 1,000 hours of diabetes care and education experience within the past five years.
  • A minimum of 200 hours completed in the past year.

Certification:

  • Certified Diabetes Care and Education Specialist (CDCES) from the National Certification Board for Diabetes Care and Education (CBDCE).
    Additional Requirements:
  • Strong computer, organizational, and communication skills.
  • Commitment to ongoing professional development and certification renewal.
