Primary Care Patient Navigator (Full-Time)

Department: Patient Navigation
Location: Jamaica Hospital Medical Center – Queens, NY
Schedule: 37.5 hours per week
Salary: $26.45 – $29.05 per hour

Jamaica Hospital Medical Center, a member of the MediSys Health Network, is seeking compassionate and motivated individuals to join our Patient Navigation Department as Primary Care Patient Navigators.

In this role, you will serve as a liaison between the MediSys Health Care System and patients, helping individuals access care, coordinate appointments, and navigate healthcare services.

Key Responsibilities

  • Conduct outreach to patients
  • Identify missing or overdue medical services
  • Assist patients with scheduling internal and external appointments
  • Help resolve barriers to appointment compliance
  • Communicate with physicians regarding patient care needs
  • Provide information on healthcare resources and patient rights
  • Participate in patient education and support groups
  • Maintain accurate documentation in electronic medical records

Requirements
  • Associate’s degree required (Bachelor’s preferred)
  • Minimum 3 years of community health or patient outreach experience
  • Strong computer skills (EMR systems, Excel, Word, PowerPoint)
  • Bilingual/multilingual preferred (Spanish, Hindi, Bangla, or Haitian Creole)
  • Strong communication, organizational, and customer service skills
  • Cultural competency working with diverse communities
  • QMI certification a plus
