  • Maintains hospital and governmental policies involving Patient’s Bill of Rights and confidentiality.
  • Maintains active social work caseload, including but not limited to: distributing mental health screenings to enrolled population; individual and family counseling; counseling services via telehealth; classroom workshops and parent workshops; and attendance at DOE meetings.
  • Performs, trains and monitors social workers to distribute mental health screening tools to the clinic’s enrolled population.
  • Performs, trains and monitors social workers to complete and follow up on any borderline or positive mental health screenings at both the elementary school and high school.
  • Performs, trains and monitors social work referrals for outpatient services to the JHMC outpatient mental health clinic, the Advanced Center for Psychotherapy, or other neighboring clinics for further treatment and follow-up.
  • Performs, trains and monitors social workers’ psychosocial assessment of students 
  • Performs, trains and monitors social workers’ crisis intervention, counseling, and support to family and students.
  • Provides, trains and monitors social work staff on age-appropriate reproductive health counseling to students, including HIV counseling and referral.
  • Performs, trains and monitors social workers’ documentation of positive screenings and any interaction with students/parents, DOE personnel, and other collaterals in EMR.
  • Performs, trains and monitors maintenance of a mental health chart in EMR for those students who are receiving counseling, which includes but is not limited to: NP/DOE referral form, psychosocial assessment, initial treatment plan, treatment plan review(s) (when indicated), progress notes, case management notes, and discharge summary.
  • Performs and monitors that data is accurately input into EMR within the specified time frame for completion and in accordance with job requirements.
  • Accurately input and train/monitor Social Worker’s completion of required entries into OSCR, an online data reporting system utilized by the Office of School Health.
  • Performs, trains and monitors the identification of all students who do not have health insurance or whose health insurance information is unknown; refers these students/families to HealthFirst and JHMC Financial Investigator, and follows up with the family/guardian until insurance information is obtained.
  • Performs, trains and monitors social workers in the high school to identify and enroll students in the Family Planning Benefit Program (FPBP) and to refer to JHMC Financial Office.
  • Performs, trains and monitors attendance at DOE meetings (e.g., parent/teacher conferences, PPT, Safety, Attendance, etc).
  • Performs, trains and monitors appropriate outreach to increase enrollment in clinics.
  • Performs, trains and monitors completion of the mental health daily logs and statistical data, as well as submission of quarterly and cumulative statistical reports.
  • Coordinates weekly supervision to discuss all social workers’ cases, which includes assessment, diagnosis, and treatment planning.
  • Performs, trains and monitors weekly meetings with medical team and daily huddles.
  • Coordinates principal meetings periodically to improve functioning of the SBHC.
  • Performs, trains and monitors social workers’ collaboration with DOE to identify appropriate school liaisons that will assist in coordinating appropriate follow-up for all enrolled students.
  • Performs, trains and monitors social workers to explore support systems (i.e. family, friends, significant others) to determine the level of involvement and dynamics.
  • Performs, trains and monitors social workers’ knowledge of community resources and appropriate referrals to meet student’s needs.
  • Performs, trains and monitors social workers’ identification and appropriate actions taken in cases including child abuse/ neglect, domestic violence, and sexual assault. Completes and monitors referrals to ACS when appropriate and conducts and monitors follow-up with ACS caseworkers.
  • Assesses and monitors assessments for suicidal ideation, intent and plan utilizing the C-SSRS. Assesses and monitors assessments for homicidal ideation, intent and plan. Completes and monitors completion of relapse prevention plans with students when indicated.
  • Performs, trains and monitors social workers’ ability to identify issues related to smoking cessation and/or substance use (with patient or family).
  • Performs, trains and monitors social workers’ ability to relate to age groups appropriate to the SBHC population and is knowledgeable of age specific developmental tasks/challenges and issues related to the age groups.
  • Assists in registration of patients in EMR and in coordination of CQI project.
  • Performs unrelated duties in times of emergency and other duties as assigned..
  • Reports to Program Director and Chief of Psychiatric Social Work regarding all administrative and clinical issues.
  • Reviews Social Work Policy Manuals (Elementary and High School) annually and updates policies and creates new policies when appropriate.
  • Exhibits cultural competence in interactions with the school’s student population.
  • Completes annual employee evaluations for all social workers in the School Health Program.
  • Conducts chart reviews for social work charts biannually.·
Requirements

1) Must have LCSW.

 2) Bilingual in English and Spanish highly preferred.

 3) Experience working with children 3-21 years of age.

 4) Fullly able to contact ACS and refer potential patients to hospital ER.

 5) Able to mentor and teach LMSW.
