Patient Navigator – Hospital Medical Home
The primary responsibility of the Patient Navigator is to act as a liaison between MediSys Health Care System and the patient. Patient Navigators are expected to provide continually exceptional customer service to the patient population and MediSys colleagues; outreach to the patient population; identify missing medical services and assist with the scheduling of these services; identify and problem solve barriers to keeping appointments; inform physicians of patients’ missing medical services, and assist patients’ on-site if needed. Additionally, Patient Navigators will be asked to participate in the Diabetes Prevention Program as a Lifestyle Coach, providing weekly educational support in a group setting to pre-diabetic patients.
