ONCOLOGY 

MONDAY- FRIDAY 8:00 AM – 4:30 PM OR 8:30 AM – 5:00 PM

The Care Management Department at Jamaica Hospital Medical Center (JHMC) assists people who need frequent medical attention. We work closely with primary care physicians (PCP), patients, families, and health care providers to support care in a patient-centered, community-based way. All our chronic care management programs strongly support shared decision-making with the patient and caregiver/family, giving patients a voice in their own health care.

Our Care Management Department has exciting opportunities for a Nurse Liaison with prior Oncology experience to join our team!

Our Nurse Liaisons provide Care Transitions and Complex Care Management services to Oncology patients with a history of complex clinical diagnoses or concerns and develop interventions to address effective self-management and avoidance of unnecessary hospitalizations.

WHAT YOU WILL DO:

In this highly visible and rewarding position, through the management of a person-centered care plan based on the patient’s needs, you will develop and nurture professional relationships with patients, families, caregivers, physicians, clinicians, and community-based organizations in the patient’s community. Additionally, you will educate staff, patients, and caregivers/families regarding their health conditions, lifestyle management, services provided by our hospital, and community resources in order to help patients reach their wellness goals. 

Requirements

TO BE CONSIDERED, YOU’LL HAVE:

  • Bachelor of Nursing degree.
  • NYS Registered Nurse license.
  • 1 year previous RN Oncology experience.
  • Bilingual (Spanish, Mandarin, Cantonese) preferred.
  • Complex Case Management and patient advocacy experience preferred.
  • Experience in assessment and care plan development.
  • Proficiency in PC and EMR documentation.
  • Excellent communication skills, both written and verbal.
  • Must have NY State Driver’s License, and vehicle and be willing to make community home visits.
