We offer comprehensive services for short-term care, rehabilitative, medical, and social services. We are currently seeking experienced Licensed Practical Nurses for immediate F/T Day  &  Per Diem Day shift opportunities.

RESPONSIBILITIES:

A licensed Practical Nurse (LPN) will provide daily hands-on healthcare to patients in our Nursing and Rehabilitation Center. Additional duties will include:

• Developing a plan of care to meet residents’ needs in collaboration with the head nurse/charge nurse
• Assuming the role of the nurse in charge as assigned by the Clinical Care Coordinator or Nursing Care Coordinator,
• Assisting ailing patients in daily necessary activities such as eating, bathing etc.
• Transcribing, updating and implementing treatment and medication orders.
• Developing assessment plans, interventions, and evaluation of patient care and response in medical records
• Obtaining and recording pertinent data in admission and nursing database
• Participating in unit-based Quality Assurance activities
• Performing the role of a patient advocate
