SENIOR CARE COORDINATOR – CARE MANAGEMENT 
Schedule: Monday – Friday, 8:30 AM – 5:00 PM

Are you seeking a rewarding career that presents daily challenges while allowing you to make a meaningful impact in patients’ lives? Jamaica Hospital Medical Center is looking for a Senior Care Coordinator to join our Care Management Department in a full-time capacity. This role provides an opportunity to use your expertise to offer personalized support to patients in the community.

Key Responsibilities:

  • Collaborate with healthcare professionals to assess patients’ physical and mental conditions, and facilitating that their discharge needs are met.
  •  Provide care transition and complex care management services to high risk patients with behavioral health diagnoses.
  •  Act as an advocate for patients, addressing crises and implementing appropriate interventions.
  • Assist patients in accessing financial entitlements, social services, and community resources.
  • Conduct biopsychosocial assessments to evaluate patients’ functioning, support systems, and current needs, developing person-centered care plans.
  • Connect patients to community-based programs to address complex health and social needs.

Requirements
  • New York State LMSW -required
  • LCSW preferred.
  • Experience in care coordination, behavioral health, psychiatric services, or substance use disorders is a plus.
  • Ability to travel within Queens and Brooklyn for home/community visits occassionally when needed.
  • Must have own car. Travel reimbursement provided.
  • Strong knowledge of medical terminology and the roles of healthcare team members.
  • Deep understanding of the psychosocial aspects of illness and health.
  • Familiarity with community resources and social service programs.
  • Excellent interpersonal, communication, and time management skills.
  • Ability to thrive in a fast-paced work environment.
  • Be comfortable working primarily telephonic.
  • Bilingual a plus
