The Care Management Department at Jamaica Hospital Medical Center (JHMC) assists people who need frequent medical attention. We work closely with primary care physicians (PCP), patients, families and health care providers to support care in a patient-centered, community-based way. All of our chronic care management programs strongly support shared decision making with the patient and family, giving patients a voice in their own health care.
Our Care Management Department has exciting opportunity for a Community Care Assistant to join their team! Our Community Care Assistant will provide Care Transitions and Care Management services for patients in the community following a recent hospitalization. Additional responsibilities include:
- Provide assessment and formulation of a person-centered care plan and engages with patients and caregivers to meet goals that matter to the patient.
- May be required to travel to patients’ homes and community agencies, including medical and/or government offices.
