Shift Hours Available: Mon-Fri, rotate every other Saturday, 7:30AM – 4PM, 8:30AM – 5PM, 12PM – 8:30PM
**2 Month training onsite/Then assigned to one of our Medisys Clinics**

Jamaica Hospital Medical Center a member of the Medisys Health network currently seeks caring, compassionate individuals for immediate full-time Primary Care Patient Navigator opportunities in our Patient Navigation department.
As a Patient Navigator, you will act and serve as a liaison between MediSys Health Care System and the patient. Patient Navigators are expected to provide continually exceptional customer service to the patient population and MediSys colleagues.

Additional responsibilities include:

  • Conducting outreach to patients.
  • Identifying missing or overdue medical services.
  • Assisting with scheduling patient internal and external appointments and services.
  • Investigating, identifying and resolving problem barriers to appointment compliance.
  • Informing physicians of patients’ missing medical services.
  • Advising and assisting individuals with information regarding resources and patient rights.
  • Participating in patient educational support groups.

Requirements
  • A minimum of an Associates’ degree is a minimum.
  • At least 3 years of experience in community health.
  • Strong computer skills (EMR systems, Excel, Word, Power Point,). Bilingual / Multilingual (Spanish, Hindi, Bangla or Creole) required.
  • Highly motivated, detail-oriented and task-driven.
  • Understanding of cultural competency common to the community.
  • QMI (Qualified Medical Interpretation) certification a plus.
