JAMAICA HOSPITAL MEDICAL CENTER, a major healthcare facility located in Queens, NY, is seeking a Case Manager RN.

About the Position: We are looking for a skilled and compassionate Case Manager RN to join our dynamic team. This role offers a starting pay rate of $50.08 per hour and comprehensive benefits through the 1199 Union, including health, dental, vision, and retirement plans.

Key Responsibilities:

  • Conduct comprehensive assessments of patients’ needs, including medical, psychological, and social factors.
  • Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams.
  • Monitor and evaluate patients’ progress and make necessary adjustments to care plans.
  • Facilitate effective communication between patients, families, and healthcare providers.
  • Advocate for patients’ needs and ensure they receive appropriate services and resources.
  • Document and maintain accurate records of care activities and outcomes.

Requirements
  • Valid RN license in New York State.
  • Bachelor’s degree in Nursing Previous experience in case management or related field preferred.
  • Strong organizational and communication skills. Ability to work collaboratively in a fast-paced environment.
