SOCIAL WORKERS –  NURSING HOME

Since 1975, Jamaica Hospital Nursing Home a 228-bed state-of-the-art facility has provided Queens’s residents a comfortable environment, with access to numerous luxurious services and amenities. We offer comprehensive services for short-term care, rehabilitative, medical, and social services. From its onset, Jamaica Hospital Nursing Home has built a reputation on quality and continues today to be one of the finest nursing and rehabilitation centers in Queens, We are currently seeking caring, dedicated, and compassionate Social Workers for immediate full-time opportunities.

DUTIES:

• Providing short term goal-oriented counseling services, support and assistance to residents and their families.
• Conducting thorough evaluations to identify residents’ emotional, social, and psychological needs.
  Maintaining and/or improving the resident’s ability to control everyday physical needs and mental/psychosocial needs.
• Connecting residents and families with external financial assistance, legal aid, mental health support, and transportation services.
• Developing, maintaining and implementing individualized service care plans.
• Participating in the planning of the resident’s admission, community re-entry or transfer to other facilities.
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.
