Jamaica Hospital Nursing Home, a 228-bed state-of-the-art facility in Queens, New York, is seeking compassionate and dedicated Social Workers to join our team. We offer comprehensive services, including short-term care, rehabilitative, medical, and social services, all within a comfortable and supportive environment.

Key Responsibilities:

• Provide short-term, goal-oriented counseling services to residents and their families.
• Conduct thorough evaluations to identify residents’ emotional, social, and psychological needs.
• Assist in maintaining and enhancing residents’ abilities to manage daily physical and psychosocial needs.
• Connect residents and families with external resources, including financial assistance, legal aid, mental health support, and transportation services.
• Develop, implement, and maintain individualized service care plans.
• Participate in planning for residents’ admissions, community re-entry, or transfers to other facilities.
• Engage in interdisciplinary care plan meetings to ensure the social and emotional needs of residents are addressed.
