RN Case Manager

Flushing Hospital Medical Center is a 293-bed, not-for-profit teaching hospital. The hospital offers a variety of services for a culturally diverse community, including Bariatric Surgical Service, Ambulatory Care Center, Women’s Health Center, Wound Care Center, and Chemical Dependence Unit. Our busy on-site full-service pharmacy provides prescription services to discharged, emergency department, and clinic patients as well as employees. 

DUTIES:

• Each Case Manager is responsible for supplying Managed Care insurers with documentation supporting acute inpatient hospitalization for  assigned patients;
• Identifying and coordinating the need for post-discharge services for assigned patients.
• Effectively collaborating with interdisciplinary team members to ensure safe and timely patient discharge.
