Care Management Nurse – Population Health Ambulatory Care

Pay: From $53.17 per hour | Full-Time | On-site

Jamaica Hospital Medical Center (JHMC), a nationally recognized leader in patient care, is seeking a dedicated Care Management Nurse to join our Population Health and Ambulatory Care team. This department focuses on improving community health outcomes by integrating care across outpatient and community settings, addressing health disparities, and promoting chronic disease management.

Position Summary:

As a Care Management Nurse, you will support patients with complex health needs through transitions of care, chronic condition management, and preventive interventions. Your role will involve coordinating services, reducing avoidable readmissions, and empowering patients to manage their health effectively.

Key Responsibilities:

  • Develop and implement individualized care plans for high-risk patients
  • Provide follow-up outreach after hospital and emergency department discharges
  • Support medication reconciliation and adherence
  • Coordinate referrals, specialist visits, home care, and social support services
  • Monitor patient recovery and update care plans accordingly
  • Educate patients and caregivers on self-management and follow-up care
  • Collaborate with multidisciplinary teams to ensure seamless, patient-centered care
  • Conduct community and home visits as needed

Requirements
  • Active NYS Registered Nurse (RN) license (required)
  • Bachelor of Science in Nursing (BSN) (required)
  • Minimum of 2 years of clinical nursing experience (hospital or managed care preferred)
  • Certification in Care Coordination and Transition Management (CCTM) preferred
  • Proficient in Electronic Medical Records (EMR) and Microsoft Office
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work in a fast-paced, team-oriented environment
  • Willingness to travel within the community when needed
