Requirements
Required Skills/Experience: Care management and patient advocacy skills Familiarity traveling around the community Knowledgeable and investigative about financial entitlements, social and community resources Excellent communication skills, both written and verbal Experience working with diverse populations and addressing complex psychosocial issues. Skills: Strong assessment, counseling, and crisis intervention skills; Excellent communication and interpersonal skills; Ability to work independently and as part of a multidisciplinary team; Proficiency in electronic health records (EHR) and case management software. Qualifications: Minimum of 2-3 years of experience in a healthcare setting, preferably in ambulatory care or outpatient services. Master’s Degree in Social Work Must Have New York State Licensed Clinical Social Worker, LCSW-C, LCSW, LMSW (LCSW-C preferred) Prior hospital or clinic experience a plus
