Patient Navigator

Description:

Flushing Hospital Medical Center a member of the Medisys Health network currently seeks caring, compassionate individuals for immediate full-time Primary Care Patient Navigator opportunities in our Patient Navigation department.  

 As a Patient Navigator, you will act and serve as a liaison between MediSys Health Care System and the patient.  Patient Navigators are expected to provide continually exceptional customer service to the patient population and MediSys colleagues.   Job responsibilities include:

• Conducting outreach to patients. 
• Identifying missing or overdue medical services.
• Assisting with scheduling patient internal and external appointments and services.
• Investigating, identifying and resolving problem barriers to appointment compliance.
• Informing physicians of patients’ missing medical services. 
• Advising and assisting individuals with information regarding resources and patient rights. 
• Participating in patient educational support groups
