The Coordinator is responsible for the overall management of the MDS program. The Coordinator will complete MDS assessments for all Medicare program residents on admissions, readmissions and significant changes. Develop MDS schedules for the following: new admissions, 7 day – 30 days – 60 days, quarterly reviews, significant changes and annual reviews.Review residents returning from hospital. Coordinate each resident assessment according to the required schedule. Coordinate the electronic submission process. Track completion of MDS reviews. Integrate assessment information and care planning. Coordinate and lead MDS Interdisciplinary meeting. Sign and certifies the completion of the assessment.
