The Department of Psychiatry at Jamaica Hospital Medical Center offers a wide range of inpatient, outpatient and community-based services for those with mental health needs. We operate two community mobile crisis teams as part of our comprehensive emergency psychiatric services. Our team provides care to patients who are either unable or unwilling to receive continuing out-patient psychiatric services.
The Mobile Crisis team, comprised of a coordinator, certified social worker, and psychiatrist, visit the homes of both recently discharged patients, as well as newly referred patients seven days a week. The purpose of the Mobile Crisis Team is to perform initial screenings or on-going assessment and stabilization.
We currently have a full-time opportunity for a Licensed Clinical Social Worker to join our Mobile Crisis night-shift team. In this role, the LCSW will render professional casework services, as an element in comprehensive care. She/He will assist the patient and their family members with social or personal problems related to their illness, hospitalization, and readjustment to the community.
Core Responsibilities:
- Aids the patient and family to identify social, emotional, environmental problems that may impede adjustment to illness, hospitalization, and projected post discharge needs, including LEP needs.
- Provide crisis intervention, counseling and support to family and patients.
- Act as a liaison between patients and/or families with the interdisciplinary hospital staff.
- Collaborate with interdisciplinary staff in the care of the patient while hospitalized, as well as projected post hospital services for continuity of care.
- Demonstrates the ability to relate to age groups appropriate to the hospital/unit population and is knowledgeable of age specific developmental tasks/challenges and issues related to the age groups (i.e., Neo-natal, pediatric, adult, geriatric)
