SOCIAL WORKER (MSW)

We are currently seeking passionate Social Worker for our outpatient Medisys Clinic for immediate full-time opportunities in the following clinics:
• EAST NY DTC-Medisys clinic
• ST ALBANS DTC-Medisys Clinic
• SUTPHIN BLVD 90-16 Medisys clinic

Hours: 8:30am – 5pm, Monday – Friday.

A Social Worker in the outpatient clinic provides emotional, social, and practical support to children and their families during their healthcare experience. This role involves assessing the needs of patients and families, brief counseling, and helping them navigate medical, emotional, and financial challenges. Pediatric social workers collaborate with doctors, nurses, and other healthcare providers to ensure comprehensive care and advocate for resources and services that improve patient outcomes. They also assist with discharge planning, connect families to community services, and provide crisis intervention when needed. The goal is to enhance the overall well-being of both the child and their family in an outpatient setting.

Additional duties will include:
• Collaborating with other professionals to evaluate patients’ physical and/or mental condition and to assess their discharge needs
• Acting as an advocate for patients to resolve crises and provide appropriate interventions
• Investigating suspected child abuse or neglect cases and initiate referrals to State Central Registry as necessary
• Completing biopsychosocial assessment of patients to gather information about their prior functioning, support system, and evaluate their current functioning to determine a safe discharge plan.
• Referring patients to community or social service programs to meet their complex needs
