We are seeking a compassionate and dedicated Social Worker (LMSW/LCSW) to join our outpatient clinic team. This role involves assessing and addressing the social and emotional needs of patients, providing counseling, connecting them to vital community resources, and collaborating with healthcare professionals to enhance patient care.

Key Responsibilities:

• Assess patients’ social, emotional, and financial needs to identify barriers to treatment.
• Provide therapy and short-term counseling to patients and families coping with illness and stress.
• Advocate for patients by securing access to necessary healthcare services and community resources.
• Collaborate with the medical team to develop comprehensive care plans and ensure seamless care transitions.
• Assist with discharge planning, including follow-up care and home health arrangements.
• Offer crisis intervention and emotional support as needed.
• Educate patients and families on medical conditions, treatment options, and coping strategies.
