Join Jamaica Hospital Medical Center’s Center of Excellence (COE) Grant Program and be part of a multidisciplinary team delivering trauma-informed, patient-centered care to individuals and families facing complex mental health and substance use challenges.

Key Responsibilities
Conduct comprehensive biopsychosocial assessments
Provide crisis intervention, counseling, and patient advocacy
Collaborate with interdisciplinary teams to assess patient needs and develop safe discharge plans
Assist patients and families in understanding diagnoses, treatment plans, and available resources
Coordinate referrals to community-based and social service programs
Investigate suspected abuse or neglect and complete mandated reporting as required
Maintain accurate and timely clinical documentation
Support continuity of care across behavioral health services

Requirements

Master’s Degree in Social Work (MSW) required

Current New York State LMSW or LCSW license required

Experience in mental health, substance use, or community-based programs preferred

Newly licensed graduates will be considered

Knowledge of medical terminology and healthcare systems Strong interpersonal, crisis intervention, and communication skills

Ability to work effectively in a fast-paced environment Commitment to equity, access, and patient-centered care
