Social Worker (LMSW)
Multiple Departments available: NYS OMH CTI, JAMAICA ESTATES, NYS OMH CTI PSYCH CLINIC
Location: Jamaica, Queens, NY (In-Person)
Schedule: Full-Time and/or Part-Time
Compensation: Competitive hourly rate (commensurate with experience)
Position Summary
The Social Worker (LMSW) provides clinical services and care coordination to patients at Jamaica Hospital Medical Center, supporting individuals affected by mental health and substance use disorders.
Key Responsibilities
Conduct psychosocial assessments and ongoing clinical evaluations
Provide individual and group counseling services
Coordinate care with internal providers and community-based organizations
Develop, implement, and monitor treatment and discharge plans
Maintain accurate clinical documentation in compliance with regulatory standards
Participate in interdisciplinary team meetings and program initiatives
Support grant objectives, outcomes tracking, and reporting requirements
