Psychotherapist – Critical Time Intervention (CTI) Program

Jamaica Hospital Medical Center is seeking three (3) full-time Psychotherapists to join our Critical Time Intervention (CTI) Program—a time-limited, evidence-based initiative designed to support individuals with mental illness or experiencing homelessness during critical transitions.

About the CTI Program:

CTI provides intensive, targeted case management to help individuals achieve stability and integrate into the community. The program is structured around a phased approach, typically lasting nine months, with a gradual transition to community-based support systems.

Key Features of CTI:

  • Time-Limited & Intensive: Focused, high-level support during a critical transition period.
  • Community Integration: Establishes connections to housing, healthcare, employment, and social support networks.
  • Evidence-Based Approach: Proven to prevent homelessness and promote long-term stability.
  • Strengths-Based & Recovery-Oriented: Encourages self-sufficiency and resilience.
  • Collaboration & Coordination: Works closely with service providers and community organizations.

Role & Responsibilities:

  • Conduct person-centered assessments in collaboration with clients.
  • Provide community-based support and home visits to monitor progress.
  • Engage with collateral contacts (family, service providers) to strengthen support networks.
  • Assist clients in accessing housing, healthcare, employment, and essential services.
  • Facilitate crisis intervention, discharge planning, and continuity of care.
  • Ensure smooth transition from institutional care to independent living.

Requirements

Qualifications:

  • LMSW or LCSW required (Provisional License holders may be considered).
  • Hospital or medical experience preferred; new graduates are welcome.
  • Strong knowledge of community-based resources for discharge planning.
  • Ability to multi-task in a fast-paced, high-volume environment.
  • PC proficiency and strong written/oral communication skills.
  • Excellent crisis intervention skills and ability to work collaboratively.
  • Community home visits required.
