Nurse Practitioner

Flushing Hospital Medical Center a member of the MediSys Health Network is a 293-bed, not-for-profit Queens, New York based teaching hospital. Conveniently located in one of New York’s most culturally diverse communities, Flushing Hospital Medical Center offers a variety of services, including a Bariatric Surgical Service, Ambulatory Care Center, Women’s Health Center, Wound Care Center, and Chemical Dependence Unit. We are currently seeking a Psychiatric Nurse Practitoner for our Center of Excellence Grant (COE) program in the Mental Health Clinic. The COE grant funded program is for the increase of patients who are high risk for all causes including ED and hospital admissions and readmissions for psychiatric, physical health, and conditions that are exacerbated by Health-Related Social Needs (HRSNs). The program was funded to increase the intensity, frequency, and duration of medication therapy, psychotherapy, and care coordination services for patients with (HRSNs).

The Psychiatric Nurse Practitioner’s responsibilities include care coordination and management, and annual psychiatric assessment, treatment planning, medication therapy individual, family, and group psychotherapy, and discharge planning. The Psychiatric Nurse Practitioner will work closely with COE grant funded social work/psychotherapist to coordinate care for patients with psychiatric, physical health, and HRSNs challenges to prevent all-cause ED and hospital admissions and re-admissions.
