Compensation: $68.76 – $76.46/hour (based on experience)

About the Role
Flushing Hospital Medical Center, a 293-bed teaching hospital and part of the MediSys Health Network, is seeking a Psychiatric Nurse Practitioner to join our Center of Excellence (COE) Program. This grant-funded initiative provides enhanced outpatient behavioral health services for patients at high risk for psychiatric and medical hospital admissions.

You will work collaboratively with a multidisciplinary team to deliver medication management, psychotherapy, and care coordination for patients with complex behavioral and medical needs.

Key Responsibilities

• Conduct initial and annual psychiatric assessments
• Develop and monitor individualized treatment plans
• Manage medication therapy and psychiatric follow-up
• Deliver individual, family, and group psychotherapy
• Coordinate care with social workers and other providers
• Support discharge planning and patient engagement to reduce hospital visit
