We have an exciting opportunity for a Case Management Assistant on our team who will collaborate with the interdisciplinary team to assist in the implementation of the discharge plan.

Summary:

The Case Management Assistant is responsible for processing and documenting all incoming correspondence from Managed Care according to department protocols, obtaining timely approvals, and processing all inpatient denials; is responsible for coordinating all post-discharge services. This includes working closely with all members (case managers, social workers) of the interdisciplinary team to ensure a safe discharge plan; is responsible for providing administrative support to both internal and external customers. Responsibilities include addressing all incoming calls and correspondences, drafting and sending letters, scanning and copying documents; is responsible for the ordering and delivery of all DME required to ensure a safe and timely discharge for patients, as needed
Assist case managers, social workers, and other members of the interdisciplinary team in all discharge planning matters.

  • Screen telephone and fax requests. Responds to inquiries and makes appropriate referrals.
  • Input essential data into EMR systems Epic and AllScripts.
  • Utilize knowledge of community resources to refer patients to appropriate services to ensure continuity and quality of care.
  • Serve as liaison with community agencies and referral services.
  • Coordinate the arrangements for referrals for transportation, durable medical equipment, residential facility placements and other facilities as needed for all eligible patients.

Requirements
  • High School diploma or bachelor’s degree preferred
  • Great communication, critical thinking, problem-solving and interpersonal skills.
  • Excellent organization and record-keeping skills, and the ability to multitask.
  • Strong computer literacy (MS Word, Outook, Excel) required.
  • Excellent time management skills and ability to work in a fast-paced work environment
