We are seeking a Case Management Supervisor (RN) to oversee a team of Case Managers and ensure patients receive high‑quality, coordinated services. This role combines staff leadership, program oversight, and direct support in complex or crisis situations. Ideal for an experienced RN leader with strong analytical, communication, and mentoring skills.

Key Responsibilities

Staff Leadership & Supervision

  • Lead, mentor, and support a team of Case Managers.
  • Provide training, coaching, and performance evaluations.
  • Ensure staff competence, professional development, and adherence to standards.

Case Management Oversight

  • Assign and review cases; monitor case activity and care plans.
  • Ensure compliance with hospital policies, state regulations, and documentation standards.
  • Support complex case resolution when needed.

Client & Patient Support

  • Oversee client services to ensure timely and appropriate resource allocation.
  • Maintain a small caseload as required.
  • Assist with escalated patient or client issues.

Compliance & Quality Assurance

  • Review documentation for accuracy and regulatory compliance.
  • Uphold all relevant laws, regulations, and organizational policies.

Administrative & Program Duties

  • Prepare and analyze reports and program data.
  • Participate in budget oversight and program improvement planning.
  • Ensure accurate data collection and reporting (Experience with EPIC a plus).

Crisis Intervention

  • Provide guidance to staff managing crisis situations.
  • Participate in direct crisis intervention when necessary.

Requirements
  • NYS Registered Nurse (RN) license
  • Bachelor’s in Nursing required; Master’s preferred
  • Case management experience strongly preferred
  • Experience in assessment, counseling, crisis intervention, and care coordination
  • 3-5 years prior supervisory or leadership experience required.
  • Strong communication, problem‑solving, and organizational abilities
  • Ability to work independently and collaboratively
  • EPIC experience preferred
