Social Worker (LMSW) – Outpatient Clinic (MediSys – Jamaica)

Location: 149-18 Jamaica Avenue, Jamaica, NY 11435
Salary: $41.78 – $55.22 per hour (DOE)
Schedule: Includes rotating weekends & holidays
Benefits: Full 1199 Union Benefits + PTO

About Us
MediSys Family Care Center in Jamaica, NY provides essential specialty medical services, including Internal Medicine, Pediatrics, and Podiatry. We are looking for a compassionate and dedicated Social Workers  (LMSW/LCSW) to support our patients and enhance their overall well-being.

Key Responsibilities

  • Assess patients’ social, emotional, and financial needs to identify barriers to care.
  • Provide therapy and short-term counseling to help patients and families cope with illness and stress.
  • Advocate for patients by connecting them with healthcare services and community resources.
  • Collaborate with medical professionals to create comprehensive care plans and ensure smooth care transitions.
  • Assist with discharge planning, including follow-up care and home health arrangements.
  • Offer crisis intervention and emotional support as needed.
  • Educate patients and families on medical conditions, treatment options, and coping strategies.

Requirements
  •  Master’s Degree in Social Work
  •  Valid New York State LMSW or LCSW license
  •  Experience in a hospital, clinic, or medical setting preferred (new graduates welcome!)
  •  Familiarity with medical terminology and Electronic Medical Records (EMR) is a plus
  •  Strong interpersonal, crisis intervention, and time management skills
  •  Ability to thrive in a fast-paced environment
