Flushing Hiospital is seeking a Clinical Pharmacy Coordinator to join our Infectious Disease Team. The Clinical Pharmacy Coordinator will be responsible for the following:
- Serve as an Infectious Diseases subject matter expert for the facility.
- Leads and mentors all aspects of the antimicrobial management program and other clinical programs
- Develop institutional guidelines for the management of common infection syndromes
- Prepares and updates facility specific protocols and procedures
- Update antibiogram, empiric guidelines and duration of therapy recommendations on an annual basis
- Provides facility expertise on streamlining or de-escalation of drug therapy, dose optimization, and IV to PO conversion
- Provide expertise on pharmacodynamics (PD) and pharmacokinetic (PK) dosing, appropriate utilization protocols, drug restrictions, and antimicrobial management program initiatives.
- Collaborates with Infectious Diseases Physicians, Pharmacists, Infection Prevention, Clinical Laboratory and Microbiology, Informatics, Nursing and other professionals to conduct the Antimicrobial Stewardship Program (ASP).
- Develop strategies for appropriate antibiotic utilization including but not limited to antibiotic guidelines, clinical pathways, restriction criteria, policy development, new laboratory methods and formulary management.
- Establish processes to report and monitor ongoing anti-infective utilization and identified metrics showing effectiveness of ASP including but not limited to performing medication use evaluations and conducting prospective audits.
- Facilitates ongoing education to pharmacists, physicians, and other disciplines developing educational materials and providing in-services as needed.
Requirements
- Hospital Pharmacy Practice and Clinical Pharmacy Practice experience in an Infectious Diseases setting is required
- BS Pharmacy or PharmD degree required
- PGYI Residency or equivalent experience required.
- Must have excellent skills in MS Word, Excel, PowerPoint
- New York Pharmacist Licensure
- BCPS certification with ID qualifications or PGY2 preferred
