Jamaica Hospital Medical Center is seeking a passionate Senior Care Coordinator for an immediate full-time opportunity within our Care Management department. As a member of our team, you will utilize your expertise and knowledge to provide personalized support to patients in and around the community.

Required Skills/Duties:

  • Completing a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning in order to establish a person-centered care plan.
  • Collaborating with medical and behavioral health professionals towards achievement of health and wellness related goals.
  • Acting as an advocate for patients to resolve crises and provide appropriate interventions and short-term coaching/supportive counseling to patients and families.
  • Able to investigate financial entitlements, social and community resources.
  • Referring patients to community or social service programs to meet their complex needs.

Requirements
  • Master’s Degree in Social Work.
  • New York State Licensed Master Social Worker (LMSW) required; Licensed Clinical Social Worker (LCSW) preferred.
  • Care coordination, experience working with individuals with Psychiatric, and/or Alcohol/Substance use disorders a plus
  • Knowledge of medical terminology
  • Understanding the roles of the healthcare team
  • Exceptional interpersonal skills
  • Deep understanding of the psychosocial aspects of illness and health
  • Knowledge of community-based organizations and resources
  • Superb written and oral communication skills
  • Excellent time management skills and ability to work in a fast-paced work environment.
  • Must be available to drive, have own care and be available to travel within the community to patients homes, agencies, including medical and or/government office.
