Patient Care Coordinator

Flushing Hospital Medical Center, Queens NY in collaboration with the SAMHSA Grant Project has a full time position for a Patient Care Coordinator. The purpose of the project is to permanently develop the Flushing Mental Health Clinic into a Certified Community Behavioral Healthy Clinic (CCBHC) by integrating expanded services into the current Flushing Hospital Mental Health Clinic. The integrated behavioral and psychological objectives of the project include successful aging by promoting mental health and physical health wellness, promoting individual and family functioning in the community, reducing avoidable emergency room visits and inpatient admissions, and reducing the costs of care.

This is full time position for an individual who is motivated to motivate recipients to comply with treatment plans, follow up and coordinate appointments with other community providers, and track data for recipients throughout the Electronic Health Record and the SAMHSA data base so Performance Improvement projects can be completed.
