Social Worker

The Department of Social Work Services at Flushing Hospital currently seeks Licensed Social Workers for immediate full-time opportunities.  As a member of our inpatient team, you will utilize your expertise and knowledge to provide personalized support to patients of every age. You will have a chance to augment medical care by assisting patients in making positive lifestyle changes, understand the impact of their diagnosis, and coordinate complex hospital discharges. Additional duties will include:

Collaborating with other professionals to evaluate patients’ physical and/or mental condition and to assess their discharge needs
Acting as an advocate for patients to resolve crises and provide appropriate interventions
Investigating suspected child abuse or neglect cases and initiating referrals to State Central Registry as necessary.
Completing a biopsychosocial assessment of patients to gather information about their prior functioning, and support system, and evaluate their current functioning to determine a safe discharge plan.
Referring patients to the community or social service programs to meet their complex
