The primary responsibility of a Project Coordinator is to review clinic schedules on a daily basis and coordinate any patient care gap needs by identifying missing or overdue medical services and assisting with the scheduling of these services; importing new patient lists, analyzing data and facilitate reaching our organization’s targeted goals; identify and problem solve barriers to appointment compliance; create outreach plans and/or MMS rotation schedules for on-site navigation staff; keep abreast of the Member Missing Services requirements and update patient navigators, site managers and physicians of any changes regarding this initiative.  Additionally, Project Coordinators are expected to assist in training and overseeing new and existing employees either on-site (at our ambulatory clinics) or remotely from the main office.

Requirements
  • A minimum of an Associate’ Degree is preferred, but not mandatory.  Alternatively, at least three years’ experience in community health is acceptable.  Additionally, other health care certifications (e.g. Medical Assistant certification) are preferred, but not mandatory.  Ideal candidate would be bilingual / QMI (Qualified Medical Interpretation) certification a plus.
  • Strong computer skills (EMR, Excel, Word, PowerPoint)
  • Excellent communication skills in multiple languages, both oral and written
  • Ability to work effectively in a team setting.
  • Detail-oriented, task-driven and organized.
  • Ability to analyze data, foresee problem areas and provide solutions.
  • Ability to efficiently coordinate care for our patient population.
  • Ability to create action plans to meet targets/ organizational goals.
  • Medical experience preferred, but not mandatory (i.e. Medical Receptionist, Patient Navigator)
  • Familiar with community resources and hospital processes, structure, and functions.
  • Prior experience as a health educator or case manager is preferred, but not required
