The Case Management department at Jamaica Hospital currently seeks RN Case Managers for immediate full-time career opportunities.

Responsibilities:

  • Responsible for supplying managed care insurers with documentation supporting acute inpatient hospitalization for patients on their assigned units.
  • Identify the need for post discharge services as well as identify potential barriers for a safe and timely discharge.
  • Collaborate with all members of the multi-disciplinary team to provide safe and timely discharge of patients.
Requirements
  • Associate degree in Nursing required, Bachelor of Science degree in Nursing preferred
  • NYS Registered Nurse license required.
  • PRI and SCREEN certification required (may be obtained following hiring).
  • Propensity for compassion and ability to relate to patients with various needs.
  • Great communication skills and ability to articulate care plans to patients and their families.
  • Excellent organization and record-keeping skills, and the ability to multitask.
  • Sound critical thinking, problem-solving and interpersonal skills.
  • Strong computer literacy to maintain and manage case records.
