Licensed Practical Nurse – LTC/REHAB

The Jamaica Hospital Nursing Home currently seeks  Licensed Practical Nurses for immediate full-time, part-time,day, evening & night shift opportunities. Selected candidates will provide hands-on healthcare to patients in The Jamaica Hospital Nursing Home. Additional duties will include:

• Developing a plan of care to meet residents’ needs in collaboration with the head nurse/charge nurse
• Assuming the role of the nurse in charge as assigned by the Clinical Care Coordinator or Nursing Care Coordinator
• Assisting patients in daily necessary activities such as eating, bathing, etc.
• Transcribing, updating, and implementing treatment and medication orders.
• Developing assessment plans, interventions, and evaluation of patient care and response in medical records
• Obtaining and recording pertinent data in the admission and nursing database
• Participating in unit-based Quality Assurance activities
• Performing the role of a patient advocate
