CARE MANAGEMENT

The Care Management Programs at Jamaica Hospital Medical Center (JHMC) are designed to assist people who need frequent medical attention. We work closely with primary care physicians, patients, families, and health care providers to support care in a patient-centered, community-based way. Our chronic care management programs strongly support shared decision making with the patient and family.  We are currently seeking Care Coordinators – Social Workers for immediate full-time care management roles.

DUTIES:

  • Provides Care Management services to patients with history of behavioral health diagnoses or concerns.
  • Develops interventions to address high or inappropriate utilization.
  • Completes comprehensive bio-psychosocial assessment, including comprehensive chart reviews for at risk patients, in their home or by phone.
  • Provides short term coaching and supportive counseling to patients and families regarding emotional factors and behavioral factors related to their illness.
  • Refers for mental health, and/or substance use disorder services as needed; collaborates with medical and behavioral health providers.
Requirements
  • Master’s degree in social work required.  LMSW preferred.  
  • Must have prior Care management and patient advocacy experience.
  • Must have access to vehicle for occasional travel to patients’ homes, community agencies,  medical and/or government offices.
  • Knowledgeable and investigative regarding financial entitlements, social and community resources.
  • Excellent communication skills, both written and verbal
  • Work may require stooping, bending, reaching and/or stretching for files, with occasional lifting.
  • Requires manual dexterity sufficient to operate office equipment if necessary.

SALARY:  $36.04/hr. - $48.74/hr.  DOE Plus Differential for Licensed Staff
