Interpreting and evaluating physician orders to prepare, calculate, and dispense medication dosage forms.
Regularly communicating with multiple prescribers and nursing personnel to clarify and modify orders.
Documenting intervention activities.
Preparing, labeling, and dispensing medication for all hospital areas.
Entering and processing medication orders in the computerized system to ensure appropriate drug distribution.
Reviewing, maintaining, and screening medication profiles for allergies, interactions, and therapeutic duplication.
Advising prescribers, nurses, and patients regarding medication contents, therapeutic actions, utilization, and possible adverse reactions and interactions of drugs.
Compounding and dispensing Parenteral Admixtures.
Directly supervising and overseeing pharmacy technicians and interns.
Requirements
Bachelors of Science Degree from an accredited college
PharmD preferred
Currently licensed and registered as a Pharmacist in the State of New York.
