The Patient Care Facilitator provides intensive facilitation and coordination of services for patients who demonstrate a pattern of frequent inpatient utilization, with the goal of averting preventable inpatient admissions.
Core responsibilities include providing community-based visits (including home visits) and phone calls for high utilizers.
Requirements
Bachelors degree in Social Work or other health & human services related discipline preferred, although may be substituted with commensurate professional experience. Healthcare experience (3+ years) in the areas of care coordination, care management, community health work, patient navigation, and social services. Prefer candidates with experience working with patients in at least one of the following areas:
Homelessness/housing assistance
Drug and alcohol addiction
Mental illness
Child and family services
Renal dialysis case management
Bilingual (English/Spanish) a plus but not required.
Will require travel to the community and to patient homes.
