COE is a transitional case management service funded by Healthfirst insurance to address the existing critical gaps in the mental health services

The goal is to target high risk individuals enrolled in NY State Medicaid program and provide support and treatment to encourage compliance

LMSW or LCSW issued by the New York State Education Department required.
(Provisional License holders may also be considered)
Hospital/Medical experience is preferred but new grads are welcome
Knowledge of community-based organizations and resources for discharge planning
Ability to multi-task and prioritize in a fast-paced, high-volume setting.
Excellent communication, organizational, and time management skills. PC Proficiency is required. Superb written and oral communication skills
Strong crisis intervention and short-term counseling skills
