Position Summary
The Social Worker provides mobile, community-based mental health crisis intervention services as part of Jamaica Hospital Medical Center’s Mobile Crisis Unit. This role delivers rapid assessment, stabilization, short-term intervention, and linkage to ongoing care for individuals experiencing acute behavioral health crises.

Key Responsibilities
Respond to psychiatric and behavioral health crises in community and home-based settings
Conduct comprehensive psychosocial and risk assessments
Provide crisis intervention, supportive counseling, and de-escalation
Deliver case management, client advocacy, and care coordination services
Monitor, evaluate, and document client progress toward treatment goals
Provide psychoeducation to patients and family members regarding mental health conditions
Coordinate referrals to inpatient, outpatient, and community-based services
Maintain accurate and timely clinical documentation
Work collaboratively within an interdisciplinary crisis response team
