Position Overview

JHMC is seeking a highly skilled Director of Case Management & Social Work to lead and oversee our Case Management and Social Work departments. This leadership role is responsible for implementing efficient care coordination strategies, ensuring regulatory compliance, and optimizing patient outcomes through effective resource utilization.

Key Responsibilities

Leadership & Operations

  • Oversee the daily operations of the Case Management and Social Work departments.
  • Supervise, mentor, and develop case managers, social workers, and support staff.
  • Establish training programs and conduct performance evaluations.
  • Monitor key performance indicators (KPIs) and implement quality improvement strategies.
  • Manage departmental budgets, staffing, and resource allocation.
  • Ensure compliance with federal, state, and accreditation standards (CMS, NYS DOH, Joint Commission).
  • Foster a collaborative team culture focused on continuous improvement.
  • Prepare and present reports on departmental performance to senior leadership.

Case Management & Social Work Oversight

  • Develop and manage utilization review, discharge planning, and care coordination processes.
  • Ensure patients receive comprehensive case management services, including assessments, care planning, and coordination.
  • Oversee care plans addressing physical, emotional, social, and psychological needs.
  • Resolve complex cases efficiently through interdisciplinary collaboration.

Requirements
  • Education: BSN o Higher
  • Licensure:  Registered Nurse (RN) license preferred.
  • CCM or ACM certification

Experience:

  • 7-10 years of progressive leadership experience in hospital case management, social work, or care coordination.
    5+ years of leadership experience in a unionized hospital environment (Required).
  • Strong knowledge of CMS regulations, utilization management, and care transitions.
  • Skills: Proven leadership abilities, interdisciplinary team management, regulatory compliance, and strong communication skills.\
