Senior Care Coordinator

We are currently seeking a passionate Senior Care Coordinator for an immediate full-time opportunity within our Care Management department. As a member of our team, you will utilize your expertise and knowledge to provide personalized support to patients in and around the community.

Duties:

Provides Care Management services to patients with history of behavioral health diagnoses or concerns and develops interventions to address high or inappropriate utilization.
Refers for mental health, and/or substance use disorder services as needed; collaborates with medical and behavioral health providers
Completes comprehensive bio-psychosocial assessment, including comprehensive chart reviews for at risk patients, in their home or by phone
Provides short term coaching and supportive counseling to patients and families regarding emotional factors and behavioral factors related to their illness

Job Type: Full-time
Pay: $36.04 – $48.74 per hour
