Schedule: Monday – Friday, 8:30 AM – 5:00 PM

Jamaica Hospital Medical Center is looking for a Senior Care Coordinator to join our Care Management Department in a full-time capacity. This role provides an opportunity to use your expertise to offer personalized support to patients in the community.

Key Responsibilities:

• Collaborate with healthcare professionals to assess patients’ physical and mental conditions, and facilitating that their discharge needs are met.
•  Provide care transition and complex care management services to high risk patients with behavioral health diagnoses.
•  Act as an advocate for patients, addressing crises and implementing appropriate interventions.
• Assist patients in accessing financial entitlements, social services, and community resources.
• Conduct biopsychosocial assessments to evaluate patients’ functioning, support systems, and current needs, developing person-centered care plans.
• Connect patients to community-based programs to address complex health and social needs.
