Patient Navigator

Flushing Hospital Medical Center a member of the Medisys Health network currently seeks caring, compassionate individuals for immediate full-time Primary Care Patient Navigator opportunities in our Patient Navigation department.

As a Patient Navigator, you will act and serve as a liaison between MediSys Health Care System and the patient.  Patient Navigators are expected to provide continually exceptional customer service to the patient population and MediSys colleagues.

Job responsibilities include:

Conducting outreach to patients.
Identifying missing or overdue medical services.
Assisting with scheduling patient internal and external appointments and services.
Investigating, identifying and resolving problem barriers to appointment compliance.
Informing physicians of patients’ missing medical services.
Advising and assisting individuals with information regarding resources and patient rights.
Participating in patient educational support groups
