EMERGENCY MEDICINE

Our Pharmacy Department now has an immediate full-time opportunity for a Clinical Pharmacy Specialist in Emergency Medicine. Our ideal candidate will be an effective communicator, able to integrate within a professional, dedicated, and patient-centered health care team.

 The selected candidate will be responsible for providing pharmaceutical care to outpatients managed by the Emergency Department Clinicians and admitted patients in the Emergency Department.  Such duties will encompass and include the assessment and care of ED patients. Evaluating, monitoring, and modifying drug therapy.  Providing recommendations to optimize patient outcomes and medication verification. Participating in procedures utilizing high-risk medications and resuscitations. Developing and implementing policy and procedures and medication use protocols and guidelines for ED patients including optimization of effectiveness, minimization of safety issues, and adverse events. 

RESPONSIBILITIES:

  • Participate as a member of the multidisciplinary Emergency Department team to provide pharmacotherapeutic management and pharmacokinetic monitoring for all current Emergency Department and admitted patients.
  • Provide pharmacy review of high-risk medication orders prior to administration.
  • Identify adverse drug events (ADEs) and assist in their management, prevention, and reporting, and develops process improvements to reduce drug errors and preventable ADEs
  • Providing patient-specific medication use teaching for discharge medications.
  • Facilitating proper information transfer regarding medication use for patients converted to inpatient registry from ED.
  • Collaboratively working with other clinicians and health care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
  • Participating in the development of medication management programs within the institution, including clinical guidelines, critical pathways, disease management, and drug use programs.
  • Providing didactic and experiential training in clinical pharmacy for PharmD students and clinical pharmacy residents.
  • Overseeing hospital Emergency medicine investigational drug program.
  • Participating in the professional development and competency of clinical staff.
  • Maintaining awareness of contemporary trends in the profession through the professional literature and regular attendance at professional meetings, institutes, and seminars
  • Collegially participating in the development of new department programs, services, and practices.
  • Documentation of all activities/interventions for cost savings and outcome purposes
  • Actively participates on the Critical Care Committee, Code PI Committee, Antibiotic Stewardship, and the Pharmacy, Therapeutics, and Nutrition Committee (P,T&N). Participating in Performance Improvement Committee and drug use evaluation program.
  • Performing formulary evaluation and class reviews for emergency and intensive care-related medications.
  • Precepting Pharmacy students for hospital clinical rotations.
Requirements
  • Doctor of Pharmacy Degree from an accredited College of Pharmacy
  • Completion of PGY1 Pharmacy Practice residency
  • Completion of PGY2 Specialty residency in Emergency Medicine
  • Skills in the multidisciplinary team involved with direct patient care competency in Emergency Medicine drug therapy desired.
  • 2 years of clinical experience preferred.

REQUIREMENTS

License/Certifications:

  • New York State Pharmacists License, or eligibility to sit for the exam
  • Board Certification in Pharmacotherapy (BCPS) or Board Certification in Critical Care (BCCCP) is preferred.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) are required (American Heart Association).
