Diabetes Educator

Responsibilities:

  • Conduct diagnostic evaluations to assess patient needs and develop individualized education plans
  • Provide education and training to patients and their families on various topics related to their condition
  • Collaborate with healthcare professionals to ensure coordinated care and effective patient outcomes
  • Monitor patient progress and adjust education plans as needed
  • Document patient interactions and maintain accurate records

Experience:

  • Previous experience in patient education or a related field is preferred
  • Knowledge of tube feeding and patient assessment is highly desirable
  • Strong communication skills, both verbal and written
  • Ability to work independently and as part of a multidisciplinary team
  • Excellent organizational skills and attention to detail
Requirements
  • Certified Diabetes Educator with a minimum of 3-5 years experience
  • Bachelor's Degree in Nursing, required.
  • Current license to practice as a Registered Professional Nurse in New York State, required.

OR

  • Bachelor’s degree in food, Nutrition or Dietetics, required.
  • Registered Dietitian, required.
