Jamaica Hospital Medical Center is a 404-bed non-profit community-based hospital and Level 1 Emergency treatment facility.  We are currently seeking a Critical Care Clinical Pharmacy Specialist for an immediate full-time opportunity in our Queens, New York facility. 

Selected candidate will be responsible for the provision of pharmaceutical care to patients managed by intensive care services involving the Medical Intensive Care Unit, Coronary Care Unit, Surgical Intensive Care Unit, and Intermediate Care Unit. Activities include participation in rounds: evaluating, monitoring, and modifying drug therapy; providing recommendations to optimize patient outcomes and medication verification. The pharmacist will also work to develop and implement medication use protocols or guidelines as they pertain to critical care (Medical, Coronary, Surgical, and Neurocritical) patients including optimization of effectiveness, minimization of safety issues, and ensuring cost-effectiveness. The pharmacist should be an effective communicator and be able to integrate themselves within the health care team and the current structure of the pharmacy department as well as document clinical activities.

RESPONSIBILITIES:

  • Practice with other members of the critical care team and focus on the needs of critically ill patients
  • Round as a member of the multidisciplinary critical care team to provide pharmacotherapeutic management for all ICU patients
  • Prospectively evaluate all drug therapy for appropriate indications, dosage, drug interactions, and drug allergies; monitor patients’ pharmacotherapeutic regimen for effectiveness and ADEs; and intervene as needed
  • Identify ADEs and assists in their management and prevention, reporting and developing process improvements to reduce drug errors and preventable ADEs
  • Provide pharmacokinetic monitoring when a targeted drug is prescribed
  • Provide drug information and intravenous compatibility information to the ICU team and nursing staff
  • Provide drug therapy-related education to ICU team members
  • Document clinical activities that include, but are not limited to, disease state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education, and other patient care activities
  • Provide didactic lectures to health professional residents and students in critical care pharmacology and therapeutics, where applicable
  • Act as a liaison between pharmacy, nursing, and the medical staff to educate health professionals regarding current drug-related procedures, policies, guidelines, and pathways
  • Participate in training pharmacy students, medical residents, and pulmonary fellows through experiential critical care rotations, where applicable
  • Develop drug monographs on issues related to drug use in the ICU
  • Implement and maintains departmental policies and procedures related to safe and effective use of drugs in the ICU
  • Collaborate with nursing, medical staff, and hospital administration to prepare the ICU for The Joint Commission survey and responds to any deficiencies identified
  • Provide consultation to hospital committees, such as Pharmacy Therapeutics and Nutrition, when critical care pharmacotherapy issues are discussed
  • Identify how drug costs may be minimized through the appropriate use of drugs in the ICU and through the implementation of cost-containment measures
  • Participate in quality assurance programs to enhance pharmaceutical care
Requirements

QUALIFICATIONS:

  • Doctor of Pharmacy Degree from an accredited College of Pharmacy
  • Completion of PGY1 Pharmacy Practice residency
  • PGY2 Specialty residency in Critical Care or equivalent Critical Care experience

EXPERIENCE AND/OR SKILLS REQUIRED:

  • 2 years of clinical experience preferred

LICENSURE/CERTIFICATION:

  • New York State Pharmacist License, or eligibility to sit for the exam
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS)
  • Board Certification in Pharmacotherapy (BCPS) or Critical Care (BCCCP) preferred
