Clinical Supervisor – DSRIP

Provide clinical oversight and supervision of DISRIP projects including Health Home at Risk and Care Transitions. Clinical supervision of Health Home at Risk Clinical Care Manager, Post Discharge Facilitators and other DISRIP project clinical oversight, management and supervisory duties as assigned.
