Jamaica Hospital Medical Center has a career opportunity for a Case Managers for our Case Management Department. The Case Manager collaborates with the Interdisciplinary team to assess, plan, implement, monitor and evaluate options and services to enable the progression of patients across the acute care continuum. He or She will perform required activities for utilization management, transition planning, and identification of actual or potential quality care issues.
Responsibilities:
• Supplying Managed Care insurers with documentation supporting acute inpatient hospitalization for assigned patients on their unit; participates in denial management for assigned patients on their unit whose inpatient services have been denied;
• Collaborate with the patient/family, multidisciplinary team including physicians and other providers, ensure the patient’s progress through the acute episode of care to discharge in an efficient and cost effective manner for assigned case load.
• Facilitate implementation of a timely discharge plan and updates discharge plan in collaboration with the multidisciplinary team in accordance with patient clinical course and continuing care needs to expedite post discharge care.
• Utilize screening criteria to make appropriate referrals.

Requirements
• NYS Licensed Registered Nurse. • Graduate from an accredited nursing program, BSN preferred or enrolled in prior to hire. • Case management experience preferred. Minimum two years of acute care nursing experience required. • Two years of experience in Case Management preferred. • Excellent communication, organizational and computer skills required. • PRI and Screen Certification required.
