Jamaica Hospital Medical Center has a career opportunity for a Care Coordinator for our Oncology Program in the Care Management Department

THE CARE COORDINATOR – ONCOLOGY:

  • Provides Care Management services for patients in the community who require more complete and comprehensive psychosocial assessments, including Social Determinants of Health.
  • Formulates a person-centered care plan based on assessment findings and together with the patient/caregiver engages in interventions to meet the goals that matter to the patient.
  • Assesses eligibility for and makes referrals to community-based psychosocial, medical, and mental health supports.
  • Collaborates with external medical and mental health providers to support client compliance with and timely completion of wellness plans.

 

 

Requirements

REQUIRED SKILLS:

  • Care management and patient advocacy skills.
  • Able to drive, have own car, and ability to navigate around the community.
  • Knowledgeable and investigative about financial entitlements, and social and community resources.
  • Excellent organizational and communication skills, both written and verbal.
  • Prior experience working with Oncology clients.
  • Prior Human Services and/or healthcare experience is strongly preferred.
  • Strong computer skills and data entry experience.
  • Knowledge of Medical Terminology.
  • Ability to work both independently and in a team setting.
  • Demonstrated ability to solve problems.           

QUALIFICATIONS:

  • A bachelor’s degree in social work or a related human services field, such as psychology, from an accredited institution is required.
  • One (1) year of care management or related experience working with Oncology patients in an inpatient or outpatient Oncology clinic, practice, or organization.
  • Highly organized and detail-oriented.
  • Ability to be patient and sensitive to individuals with disabilities.
  • Excellent verbal, written, and interpersonal communication skills.
  • Must be computer literate with a working knowledge of MS Word and MS Excel.
  • Must have NY State Driver’s License, and vehicle and be willing to make community home visits.
