Jamaica Hospital Medical Center is seeking a Care Coordinator to support our Oncology Program within the Care Management Department. This role focuses on providing comprehensive care management services to oncology patients in the community, addressing both medical and psychosocial needs.

Key Responsibilities:

  • Conduct psychosocial assessments, including Social Determinants of Health.
  • Develop and implement person-centered care plans in collaboration with patients and caregivers.
  • Facilitate referrals to community-based medical, mental health, and social support services.
  • Coordinate with external healthcare providers to ensure patient compliance with treatment plans.

Requirements
  • Qualifications:
  • Bachelor’s degree in Social Work, Psychology, or related field.
  • 1+ year of experience in care management or working with oncology patients.
  • Strong knowledge of medical terminology, financial entitlements, and community resources.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to work independently and collaboratively within a team setting.
  • Proficiency in MS Word & Excel.
  • Valid NYS driver’s license and personal vehicle for community visits.
