Jamaica Hospital Medical Center is seeking a dedicated and compassionate Care Coordinator to support our Oncology Program within the Care Management Department. This vital role focuses on delivering comprehensive care management services to oncology patients in the community, addressing both their medical and psychosocial needs.

ROLE  OVERVIEW
As an Oncology Care Coordinator (OCC), you will help cancer patients navigate the complexities of their treatment by addressing their medical, emotional, and social needs. Acting as a liaison between patients, families, and providers, you will ensure a supportive and coordinated cancer journey. OCCs assist patients in understanding their diagnosis, treatment options, and potential side effects, while connecting them with essential resources such as financial aid and support groups.

RESPONSIBILITIES

  • Conduct thorough psychosocial assessments, including evaluations of Social Determinants of Health (SDOH).
  • Develop and implement person-centered care plans in collaboration with patients, caregivers, and healthcare providers.
  • Facilitate timely referrals to community-based medical, mental health, and social support services.
  • Coordinate care with external healthcare providers to promote adherence to treatment plans and ensure continuity of care.
  • Serve as a central point of contact for patients and families.
  • Provide clear, compassionate education on diagnoses and treatment plans.
  • Assist with scheduling appointments, coordinating follow-ups, and reviewing medical information.
  • Offer emotional support and guidance throughout the cancer care journey.
  • Help patients access community resources (e.g., transportation, financial assistance, support groups).
  • Identify and work to resolve barriers to care, including insurance or logistical issues.
  • Collaborate with a multidisciplinary team, including oncologists, nurses, and social workers.
  • Advocate for patient-centered care, ensuring individualized treatment approaches.
  • Contribute to quality improvement initiatives aimed at enhancing patient outcomes and care delivery.

Requirements
  • Qualifications:
  • Bachelor’s degree in Social Work, Psychology, or related field.
  • 1+ year of experience in care management or working with oncology patients.
  • Strong knowledge of medical terminology, financial entitlements, and community resources.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to work independently and collaboratively within a team setting.
  • Proficiency in MS Word & Excel.
  • Valid NYS driver’s license and personal vehicle for community visits.
