Jamaica Hospital Medical Center has a career opportunity for a Care Coordinator for our Oncology Program in the Care Management Department

THE CARE COORDINATOR – ONCOLOGY:

Provides Care Management services for patients in the community who require more complete and comprehensive psychosocial assessments, including Social Determinants of Health.
Formulates a person-centered care plan based on assessment findings and together with the patient/caregiver engages in interventions to meet the goals that matter to the patient.
Assesses eligibility for and makes referrals to community-based psychosocial, medical, and mental health supports.
Collaborates with external medical and mental health providers to support client compliance with and timely completion of wellness plans.

REQUIRED SKILLS:

Care management and patient advocacy skills.
Able to drive, have own car, and ability to navigate around the community.
Knowledgeable and investigative about financial entitlements, and social and community resources.
Excellent organizational and communication skills, both written and verbal.
Prior experience working with Oncology clients.
Prior Human Services and/or healthcare experience is strongly preferred.
Strong computer skills and data entry experience.
Knowledge of Medical Terminology.
Ability to work both independently and in a team setting.
Demonstrated ability to solve problems.

QUALIFICATIONS:

A bachelor’s degree in social work or a related human services field, such as psychology, from an accredited institution is required.
One (1) year of care management or related experience working with Oncology patients in an inpatient or outpatient Oncology clinic, practice, or organization.
Highly organized and detail-oriented.
Ability to be patient and sensitive to individuals with disabilities.
Excellent verbal, written, and interpersonal communication skills.
Must be computer literate with a working knowledge of MS Word and MS Excel.
Must have NY State Driver’s License, and vehicle and be willing to make community home visits.
